index id number between 1 and 254
routine defines how the term is extracted, the <parm> is optional and specific to the index routine.
- use “words” for basic keywords
- use “phrase2’ for basic bound phrases
fieldlist contains a list of BER tag paths specifying the fields to index
positional info list defines the positional information to associate with each occurrence of the term
