Procedure:

• Optionally, type a date for the NeedBeforeDate variable.

• Optionally, add a note for the RequesterNote variable.

• Type the email destination address in the [Emailinfo] section.

• Type the delivery address information in the [LibraryDeliveryInfo] section.

• Type the billing address information in the [LibraryBillingInfo] section.
